Important Guidelines

  • Submit clear, legible copies of all documents.

  • Blurry images or incomplete scans will be returned for re-submission (or may be rejected).

  • Upload each file individually using the link at the bottom of this message.

  • Accepted file formats: Word (.doc/.docx), PDF, or JPG.


Acceptable Proof of Identity

Choose one of the following:

  • State-issued identification card
  • State-issued driver’s license
  • Government-issued passport
  • Government-issued military ID

🚫 Do not submit Social Security cards.


Proof of Address Requirement

We must also confirm that you have permanent, stable internet service at the address where you will work. Please upload your internet bill showing:

  • Service provider name
  • Account holder name
  • Service address

The bill does not need to be in your name; however, the address must match the one listed on your application.


If Your ID Address Does Not Match Your Current Address

If your government-issued ID shows a different address than your application, you must also submit one of the following (address must match your application):

  • Utility bill
  • Cell phone bill
  • Bank statement (address page only; no personal financial info)
  • W-2 or 1099 form
  • Insurance bill

⚠️ Note: Your photo ID must be issued by the state listed on your application. If your ID is from a state not supported, we cannot proceed.
